Registration

If you are a new patient please print and complete your patient registration form and medical health history prior to your appointment or arrive 20 minutes early to complete your paperless registration through our Fast Check-in kiosk.

At your first appointment with FDH you will be welcomed by our friendly receptionists who will ensure that they have your registration, health history and any insurance information needed for your appointment. Please read through the FDH financial policy for information regarding insurance and collections.

Medical Records

Any pertinent medical and dental information along with current records that you may have from your previous dentist should be obtained prior to the appointment.

A records release form can be printed and sent to your dentist to complete this process. We typically request any bitewing x-rays taken within the last 2 years, full mouth series within the last 3 years and panorex within the last 5 years be sent to our office via email.

Your Appointment

Your first dental appointment is typically 90 minutes long. At this time your dental hygienist will review your history with you, determine your dental hygiene needs and complete your cleaning. They can also address any wants, concerns or questions you might have.

Depending on your specific needs there are some times when we are not able to complete the cleaning but we do strive to provide enough time to do so. You will also meet one of our doctors to complete your dental exam. They will review your records and the findings from the hygienist to determine if any treatment is needed.
